Using judging words	Underreporting on the most important topics	Descriptive, neutral language
Nagging about filling it in every week	Raises the cost of staying in the follow-up	Explore the non-response at the appointment, with curiosity
Reading “didn’t answer” as “isn’t adhering”	They’re different things. Some patients answer everything and don’t adhere, and the reverse happens too	Treat a non-response as a question to ask
Comparing patients by score	The scale is subjective and within-person	Compare the patient with themselves
Asking what you won’t use	It costs the patient’s attention and changes nothing	The decision test. If nothing changes, cut it

Review every 90 days

Set aside an hour a quarter and apply these pruning criteria:

Zero variance: everyone always answers the same thing. The question discriminates nothing. Cut it or rewrite it.

Never used: you’ve never once cited that data in an appointment. Cut it.

High skip rate on the optional questions: a sign of high cost or confusing wording.

Open answers revealing a misunderstanding: if the patient answered about something else, the wording is ambiguous.

How to edit in LiveClin

The platform locks editing and deleting questions once the questionnaire is saved, to preserve history and charts. You have three routes:

Hide: clear the Show option. The question disappears for respondents and the entire history is kept.

Actually edit: duplicate the question, edit the copy, and disable the original, leaving Show cleared.

Add: new questions can go in at any time, with no restriction.

Because of the editing lock, always use the preview before saving. It shows the questionnaire exactly as the patient will see it, including the result screen.
