Prerequisites

Before you start, you need:
  • A Liveclin account with access to Settings.
  • To have completed the welcome tour in getting started, where the platform’s principles are introduced.
  • At least a few questions available. See how to create and customize questions.

Step by step

Open the questionnaires screen

In the sidebar, go to Settings → Questionnaires and open Questionnaires.

Create the questionnaire and set the title

Click Add, in the top-right corner, and set a title for the questionnaire — for example, “Weekly review”.

Add a cover image

Upload the cover image, adjust the framing, and click Apply to save it.

Add the questions

Click Add question. Check the questions you want to include — you’ll see both your own and Liveclin’s predefined ones. To create a new one right there, use Create question (details in create and customize questions).Once selected, click Add. They appear listed in the questionnaire.

Check the preview

Click Preview to see the questionnaire exactly as your patient will answer it, including the result screen.

Set the weight of each question

The weight adjusts how much each question influences the final score, based on the patient’s goal. For a body-definition goal, for example, you might give weight 5 to aerobic frequency and plan adherence, and weight 4 to sleep hours and sleep quality.Well-defined weights make the final score more realistic relative to the client’s answers.

Mark what is required

For each question, choose whether the answer is required or optional. Clear the requirement where it makes sense — for example, body weight.

Review and save

Review the entire questionnaire before saving.
After you save, you cannot edit or remove the questions in the questionnaire. If you find a mistake, fix it before saving.
When everything is right, click Save. Your questionnaire is created.

After saving: what you can and can’t do

Liveclin locks editing and deleting saved questions to preserve the answer history and the progress charts, avoiding incompatibilities. Even so, you have options:
  • Hide a question: clear the Show option. The question stops appearing to respondents, but the entire answer history is kept.
  • “Edit” a question: duplicate the question, edit the copy, and disable the original (leaving Show off).
  • Add questions: you can include new questions at any time, with no restriction.
